Practice Area

Patient Addresses - Outside the Practice area

Templepatrick Surgery use the Electoral Roll to confirm the addresses of patients in the Surgery. If your address falls outside the Surgery boundary you will be advised by letter that your address is outside our Surgery area and be advised to register with a local GP.

From the 1st October 2017 if the Practice receives a letter with an address on it which is outside are Practice boundary we will write to the patient advising them that we have received the letter and assuming that they have changed address. The letter advises patients that they will need to register with a new GP as the address falls outside the Practice boundary. If the patient has not moved address they are asked to provide two forms of address confirmation dated from the date on the letter. These documents have to be provided within 14 days. After the 14 days BSO will be notified of the patients change of address and asked to remove the patient from our Practice list.
